STEP 2
In the "Other Information" section of the order form let us know

(1) which product;
(2) how many bottles and 
(3) how often you wish to receive these products (1, 2, 3, 4, 5 or 6 months. Shipping interval cannot exceed 6 months).  

PLEASE NOTE: if you do not include this information, then we will ship the same order every two months. 

When we receive your order form, we will ship your initial order immediately (with 20% discount) and we will automatically ship your repeat order as often as you have indicated on the form.
